How do I know a tooth is cracked?
The distinguishing sign is a sharp, brief pain when you bite down and especially when you release the bite — quite different from a constant ache. Sensitivity to cold on one specific tooth is common too. Cracks are often invisible on an X-ray, which is why the exam matters.
Can every cracked tooth be saved?
No, and where the crack runs decides it. A chip in the enamel may need only smoothing or a bonded filling. A crack reaching the nerve needs a root canal first. A crack running below the gum that splits the root usually cannot be saved.
Will I need a crown?
Often, yes. A crown holds a cracked tooth together and stops the crack spreading under bite pressure — a filling sits inside the tooth and does not do that. Front teeth with small chips are frequently the exception.
It does not hurt much. Can it wait?
Cracks propagate. A tooth that could be crowned today may not be salvageable in a few months, and once a crack reaches the root the option set narrows to extraction. A painless crack is worth assessing sooner rather than later.
A cracked tooth is a diagnosis before it is a repair
'Cracked tooth' covers a wide range, and the repair depends entirely on where the crack runs. A chip in the enamel may need nothing more than smoothing or a bonded filling. A crack that reaches the nerve means a root canal before the tooth can be restored. A crack that runs below the gum and splits the root usually cannot be saved at all.
Which is why the first appointment is about finding out which of those you have, not about committing to a treatment. The distinguishing symptom is worth knowing: a sharp, brief pain on biting down — and especially on releasing the bite — points to a crack far more specifically than constant aching does.
