Why Do My Gums Hurt in One Spot? Causes and Next Steps

The Usual Causes of a Single Sore Spot

• Food or floss stuck under the gum. Meat fibres, popcorn husks and shredded floss lodge between two teeth. The ache is sharp, sits in one place and often eases the moment it is removed.
• A small injury. A sharp chip, a hard crust or a burn from something hot leaves a raw patch that stings for several days.
• A wisdom tooth coming in. The gum flap over it traps bacteria and swells. See pericoronitis.
• Localised gum disease. One tooth can have a deep pocket while the rest of the mouth is fine.
• An infection at the root. Pus looks for a way out and can form a pimple on the gum. Our post on the gum boil explains what that means.

How to Tell a Gum Problem From a Tooth Problem

Look at what the spot is doing, not only how it feels.

• A red, tender gum edge next to a tooth, with no lump, is usually the gum itself.
• A small pimple or bump that comes and goes, sometimes with a salty taste, points to an infected tooth. It can be painless, as in a gum boil that does not hurt.
• Pain when you bite or tap that tooth, or a tooth that is more sensitive to hot and cold, points to the tooth rather than the gum.
• A gum problem that only ever affects one tooth deserves an x-ray, because the cause can be the nerve or the bone. This is the gum abscess that does not start in the nerve.

If the swelling is obvious, read swelling around one tooth and a painful lump on the gum.

What You Can Do for a Day or Two

• Rinse with warm salt water, half a teaspoon in a glass, a few times a day.
• Floss gently once around the sore spot to clear anything trapped. If it makes the pain much worse, stop.
• Brush the area with a soft brush, gently, along the gum line.
• Use an over-the-counter anti-inflammatory if you can normally take one, following the label.

Do not squeeze a pimple on the gum and do not put aspirin against it. A trapped piece of food should ease within a day of coming out. A sore spot that has no clear cause and is still there after two or three days needs a look.

Signs That Should Not Wait

• Swelling of the face, cheek or under the jaw.
• Fever, or feeling unwell along with the gum pain.
• Difficulty opening your mouth or swallowing.
• Pus you can taste, or a bump that keeps returning.

What Happens at the Visit

We look at the gum, press on the teeth beside it, measure the pockets around them and take an x-ray of the tooth involved. Trapped debris is cleared. Localised gum disease is treated with a deep clean of that area. If the cause is an infected tooth, we talk first about saving it, often with a root canal. And if it turns out to be a scratch from a crust of bread, we say so and send you home.
