Awake at 2am With a Throbbing Molar

Quick answer: A toothache that throbs, keeps you awake, lingers after hot or cold, and no longer responds well to painkillers is irreversible pulpitis. Antibiotics will not fix it. The only thing that reliably stops the pain is removing the inflamed nerve, and it can usually be done the same day.

The call

Yehuda, 36, from RBS Daled, messaged us at 6:15am after his third consecutive night awake. It had started four days earlier as sensitivity to cold. By night two it was a deep, rhythmic throb on the lower right that got worse the moment he lay down. By night three he was sitting up in an armchair, having taken more ibuprofen than he should have, and it was barely denting the pain.

He also said something I hear constantly: he could not tell which tooth it was.

What we found

The inability to localise pain is itself diagnostic. The pulp inside a tooth has no proprioceptive nerve fibres, so a tooth with pulpitis genuinely cannot be pinpointed. Once the inflammation spreads through the root tip into the ligament — which does have those fibres — the pain becomes precisely localisable. He was not there yet.

Testing found it: the lower right first molar had a deep filling under which decay had reached the pulp. Cold caused a stab of pain that lasted well over thirty seconds after the stimulus was removed. That lingering response is the line between a nerve that can recover and one that cannot. Nothing was tender to tapping and the x-ray showed no infection at the root tips yet.

Why it is worse at night: lying flat increases blood pressure in the head, the pulp is sealed in a rigid chamber with nowhere to expand, and the pressure rises. There are fewer distractions, too, but the mechanism is real.

What we did

We saw him at 8:30am. Getting a molar in this state properly numb takes care — inflamed pulp tissue resists local anaesthetic, and a standard block often is not enough. We used a block plus supplementary intraligamentary injection and tested thoroughly before starting.

Then a full pulpectomy: rubber dam on, access opened, all the inflamed pulp removed from every canal, canals cleaned and shaped, and a medicated dressing placed with a solid temporary seal.

His pain was gone before he left the chair. That is the normal outcome, and the reason emergency root canal treatment is worth doing properly rather than sending someone away with tablets.

The canals were filled at a second visit two weeks later, and the tooth crowned six weeks after that.

What to do if this happens to you

  1. Do not wait for the morning if you can be seen sooner — but if you must wait, sleep propped up on two or three pillows.
  2. Take ibuprofen and paracetamol together, alternating, if you can take both. It works better than either alone.
  3. Do not put aspirin on the gum. It burns the tissue and does nothing for the tooth.
  4. Avoid hot drinks. Heat is usually the worst trigger at this stage; cold sometimes gives brief relief.
  5. Do not expect antibiotics to help. There is no blood supply reaching an inflamed pulp to deliver them.
