A New Oleh, a Root Canal, and No Idea What Was Covered

Quick answer: Israel's basic health basket covers almost no adult dentistry. Children under 18 are covered through the kupot for a small copayment. Adults rely on supplementary insurance (bituach mashlim), which typically gives a partial refund rather than free treatment, and you usually pay first and claim afterwards.

The call

David, 33, six weeks off the plane from Manchester and living in Ramat Shilo. Severe pain in an upper molar, no Hebrew to speak of, a kupah membership card he had been given at the airport, and no idea whether he was supposed to go to a dentist, to the kupah, or to a hospital.

He had spent two days trying to work out the system while in genuine pain. That is a bad combination and an avoidable one.

What we found

Upper left second premolar, irreversible pulpitis from decay under a filling placed in the UK. Straightforward clinically. The complexity was administrative.

Here is the map, and it is worth having before you need it.

The basic basket (sal habriut). For adults, this covers essentially nothing dental. Not fillings, not root canals, not crowns. It covers children up to 18 and, in more recent years, has extended to certain older age groups — the age brackets shift with policy, so check current entitlement rather than relying on what a neighbour tells you.

Children under 18. Covered through your kupah's own dental clinics for a small copayment. Check-ups, fillings, extractions and more. If you have children, register them and use it.

Supplementary insurance (bituach mashlim / shaban). This is the layer most adults rely on. Every kupah sells it, plans differ, and dental cover is often a separate add-on rather than automatically included. It typically works as a partial refund against a fee schedule, at kupah clinics or at private dentists depending on the plan, and there is often a waiting period after joining before dental benefits activate.

Private insurance. Sometimes held through an employer. Worth checking; people frequently have cover they have forgotten.

That waiting period is the trap for new olim. Sign up on arrival, before you need it.

What we did

Clinically: an emergency pulpectomy the same afternoon to stop the pain, the root canal completed two weeks later, and a crown after that. Explained in English, which mattered more than he expected — being able to ask questions about your own treatment in your own language is not a luxury when you are frightened and in pain.

Administratively: we gave him itemised receipts with treatment codes, checked which kupah he was with and whether he had taken the supplementary plan (he had not, and did so that week), and explained what he could and could not claim retroactively.

The outcome

The tooth was treated. He paid privately for this one, having arrived before his cover was in place, and he has since used his supplementary plan for two further treatments.

What to do if this happens to you

1. Sort out supplementary insurance in your first weeks, before you need it. Waiting periods are real.
2. Register your children with the kupah dental clinic. The under-18 entitlement is substantial and underused.
3. Ask for an itemised receipt with treatment codes. You cannot claim without one.
4. Ask the clinic directly whether they work with your kupah's plan and whether they bill directly or you claim back.
5. Do not delay treatment while you research the system. Pain relief first, paperwork second.
