A Bracket Came Off and Slid Along the Wire

Quick answer: A bracket that has come off but is still threaded on the wire is not an emergency, provided nothing is sharp. Stabilise it with wax, avoid hard food, and see your orthodontist within a few days — but do not simply wait a month, because that tooth stops moving and the treatment stalls.

The call

Thursday night in Sheinfeld. Shimmy, 14, had bitten into a schnitzel baguette and a bracket on a lower premolar had come away from the tooth. It was still on the wire, sliding back and forth, and the wire was now loose in that section.

His orthodontist could see him in eleven days. His mother phoned to ask whether that mattered.

It did — not urgently, but it mattered.

What we found

The bracket was fully debonded, with the adhesive still attached to the bracket base rather than to the tooth, and the tooth surface was clean.

Nothing was sharp and nothing was cutting the cheek. He was uncomfortable rather than in pain.

Why brackets come off. Almost always mechanical force, not defective glue: - Hard or sticky food. Baguettes, pizza crusts, apples bitten whole, ice, sweets, popcorn. - Biting on the bracket when the bite is deep and the lower teeth strike the upper brackets. - Contact from a bite plate or a habit like nail-biting or pen-chewing. - Occasionally a bonding surface that was contaminated with saliva at placement.

Shimmy's was food, and the third bracket he had lost in eight months — which itself is information. Repeated debonding at the same site usually means the bite is knocking it, and that needs addressing rather than simply rebonding again.

Why waiting matters. Orthodontic movement is continuous. A tooth without an attached bracket receives no force; meanwhile the teeth on either side keep moving. Over weeks, that tooth is left behind and the alignment achieved so far is partly lost. One or two weeks is generally recoverable. A month or more often adds time to the overall treatment.

What we did

Two options, and we explained both.

Stabilise and refer. Secure the loose bracket in position with wax so it cannot slide or rub, and let the orthodontist rebond it. Appropriate when the orthodontist can see them soon.

Rebond it locally, with the orthodontist's agreement. Many general practices can rebond a straightforward bracket. The position has to be accurate — bracket placement determines tooth position, and a bracket bonded a millimetre off produces a tooth in the wrong place.

We phoned his orthodontist's office the next morning. They confirmed the bracket position from their records and asked us to rebond it, which we did: cleaned the tooth, etched, bonded, positioned the bracket using the height gauge measurement they gave us, cured, and re-engaged the wire.

Twenty minutes, and he lost no treatment time.

The outcome

Treatment finished on schedule. We also identified that his deep bite was causing the lower brackets to be struck by the upper teeth, and his orthodontist added a bite turbo to lift the bite, after which he lost no further brackets.

What to do if this happens to you

1. Check for anything sharp. If a wire end or bracket is cutting, that needs seeing sooner.
2. Wax over the loose bracket to hold it and stop it rubbing.
3. Do not remove it from the wire yourself. If it comes off entirely, keep it.
4. Phone your orthodontist first. They may want to do it themselves or authorise a local practice.
5. Soft diet until it is repaired.
6. Do not leave it for weeks. Treatment stalls at that tooth.
